01

Who is this for?

One person or a couple (mirror POAs in one order).

Person 1 — you

02

Type of POA

Enduring continues after loss of capacity. General is time-limited and ends if you lose capacity.

What does the enduring POA cover?

Most clients want both. Untick either box if you only want one type.

Time limit

03

Attorney(s)

Who you're appointing to act for you. Add a primary attorney and up to two alternates.

If couple. The most common couples set-up is each spouse appointing the other as primary, with the same alternates (e.g. an adult child or sibling). The form captures attorneys once and applies them to both POAs. Use the "Person 2 differs" textarea at the bottom of this section if Person 2 wants different attorneys.
Primary attorney
Alternate attorney steps in if primary can't act
Second alternate attorney optional · further fallback

How do multiple attorneys act?

Most common: consecutive — primary acts first, alternate only if primary can't.

When can the attorney start acting?

04

Limitations and conditions

Anything the attorney cannot do, or must check with someone first.

Tick any that apply. Special conditions or restrictions beyond these may attract an additional fee — we'll confirm before drafting.

05

Supporting documents

Optional uploads.

Useful: any existing POA you'd like reviewed or revoked, photo ID, medical certificates if relevant.

Special requests, anything outside standard scope?

Existing POA to revoke, complex family circumstances, specific clauses, anything else.
